CARE TRANSITIONS COACH
Peninsula Agency on Aging (PAA) is currently
seeking an individual to serve as the Care Transitions Coach to provide support
to individuals from hospital discharge to 30 days post-discharge.

services that have enabled older adults to live independently since 1974. Our mission is to support the independence
and quality of life of Peninsula residents, primarily 60 years and over, by
advocating, arranging, or providing vital human services.

Key Responsibilities
As the Care Transitions Coach, you will provide
information and guidance to the patient and/or family for an effective care
transition, to improve self-management skills and enhance patient-practitioner
communication, by assisting patients with the development of a personal health
record; practice medication management; schedule follow-up appointments with
their physician/specialist; and learn to recognize symptoms that indicate their
condition is worsening and how to appropriately respond.
Essential
Skills and Experience
We are looking for applicants with a Bachler’s
Degree or equivalent experience in Health Care Field, Care Coordination, or
Social Work. Applicant must have ability
to work with patients/families of all ages and in a variety of settings, work
independently while collaborating with other professionals, working knowledge
of health care industry, caregiving, chronic disease management, familiar with
a wide variety of community resources, excellent verbal, written and computer
literacy. Applicants must also have the
ability to model our values and be vaccinated for COVID.

8:30 am – 5:00 pm, 40 hours/week, Pay Range:
$19.23 - $20.38
